Appendixes which are to be enclosed with the application are: excerpt from the Register of Births, excerpt from the Register of Marriages, notarized copy of the foreign travel document, and if the applicant does not have one, other corresponding documents with a photograph, certificate on nationality from which it is clear when the applicant acquired foreign nationality, excerpt from the Register of Births or excerpt from the Register of Marriages for the applicant’s parents and proof of Croatian nationality of the parents or one of the parents (certificate of nationality or a certificate from the Registrar’s Office). For persons who emigrated from Sljedeće je priloge potrebno priložiti uz zahtjev: izvadak iz matice rođenih, izvadak iz matice vjenčanih, ovjerenu presliku putne isprave, a ako ju podnositelj ne posjeduje, druge odgovarajuće isprave s fotografijom, uvjerenje o državljanstvu iz kojeg je razvidno kada je i po kojoj osnovi podnositelj zahtjeva stekao strano državljanstvo, izvatke iz matice rođenih ili izvatke iz matice vjenčanih za roditelje podnositelja i dokaz o hrvatskom državljanstvu jednog ili oba roditelja (domovnice ili potvrde matičnog ureda).
